ALICE Portal – Survey Builder Tab

This article reviews the ALICE Portal's Survey Builder tab and how to add, edit, and remove Survey Questions.

The Survey Builder tab is used to add or edit survey questions. Before building a new survey, the survey listing must be created on the Survey List tab. Once a survey listing is created, complete the questionnaire on this tab. Alternatively, select the Build button on the survey listing’s row on the ALICE Portal Survey List Tab.

Adding Survey Questions

Before creating a Survey Questionnaire, select the Survey to edit using the “Select Survey List” drop-down box. If the survey doesn't appear in the drop-down box, revisit the Survey List to verify that the Survey listing was created.

Once the Survey List is selected, type the question in the text field. Images and videos can also be embedded during this process. To require a response to a question, click the “Required” toggle.

Select “Type” from the drop-down menu, determining how the user can respond. Some response types include a "Pass" or "Fail" option. If a response is set to "Fail," the visitors who select that response will be flagged. An assigned user will also be notified. An admin must provide an override code to let the visitor complete the log-in.

  • Label – Displays a message to the visitor.  Requires no response from the visitor.
  • Text – The visitor can type in a response to the question.
  • Email – The visitor can only enter a valid email address.
  • Phone – The visitor can only enter a valid phone number.
  • Check Box – Presents the visitor with multiple responses. Visitors can select multiple checkboxes as a response.
    • Select the Add Option to create a response. Populate the field with the response connected to each check box.
  • Radio Button – Presents the visitor with multiple responses. Visitors can select one as a response.
    • Select the Add Option to create a response. Populate the field with the response connected to the selection.
  • Text Area – The visitor can provide a long-form answer to the question.
    • This feature is not recommended for offices with significant amounts of visitor traffic.
  • Signature Box – The visitor can provide their signature in response to the question.

Save the question by selecting the orange Save Question option.

Editing a Survey Question

To edit a survey question, first select a Survey List. The list of questions associated with the survey will appear. Select the question to edit.

Once finished, select Save Question.

Deleting a Survey Question

To delete a survey question, first select a Survey List. The list of questions will appear under the listing. Mouse over the survey question to show the deletion icon. Click the trashcan to delete the question.

Enabling / Disabling a New Survey

Once finished creating the survey, return to the Survey List section and change the “Publish” setting for the survey from “Off” to “On.” To disable it, set "Publish" to “Off.”
